When you challenge an existing NPC Godfather to a duel to take over their position, they may have had advance warning depending on the number of people set to Disloyal.  If the godfather has advanced notice, it greatly increases their chance of fending off your attack, delaying when you are eligible to try again or even ending in your premature death.
