Someone was probably choking for a Boss kill though. That and it appears that some people have more fun keeping everyone happy and friends than reaping the benefits of their hard earned power. I would laugh in the face of some idiot leader who decided that his inactive Gangster was worth the life of one of my Bosses. Would they really sacrifice their whole family in a futile war against Detroit for a Gangster? Of course not. Now before there is an influx of "You are not a leader so shut your rancid vag mouth...", I am well aware of this and it does not alter my opinion on the matter. Please spare yourself the energy.

Formulaic procedures always bother me. It seems so often that people cannot think outside the box. It's always "If X, then Y". Each situation should call for a different reaction, depending on individual circumstances. If the Boss of the boss was more powerful than the Boss of the inactive gangster, then that stronger Boss should indeed have told the Gangster's Boss to take a flying fuck. Of course, then the shooter should have got a verbal ass kicking behind closed doors, and whatever other punishment deemed appropriate. The point is, the reaction should not be based on a previously established set of universal guidelines for how to deal with a situation. Instead, various factors should come into play, most importantly the power of the family being imposed upon to weaken themselves.

I'm surprised I haven't noticed this till now... 

I see my father's death as a learning lesson for my bloodline. True, he fucked up big time and shouldn't have been up so late in LA shooting IA's. He was a very careful man who took his time with each of his shots. The one time he rushed and only checked to make sure he was an IA was his downfall...

I leaned a very important lesson through the loss of my father. I think it's far more ingrained in me how important it is to be fully aware than if my father was able to pay off a blood debt and go on his way. 

True, I was frustrated at first but I think it's better the current way we do things. You don't have many high-rankers making such mistakes because they know the consequences and may have learned from the same lesson.

Judge, I respect your opinion and I can see exactly what you are saying. However, I completely disagree that every CL has to be involved in the decision making process. In these situations, the other cities likely couldn't care less what resolution is made between two families in cases like these, nor should they, it's not their concern.
